Fewer crisis points in daily life.

When eldercare is planned and supported by a team with deep geriatric experience, the result is stable, healthier lives.

Exceptional care is only available from experts.

Since 2002, as co-founder of Home Care Assistance, Dr. Kathy Johnson has long crusaded for better quality care in the senior services industry. Shelterbelt Health raises the bar with expert leaders setting the highest standard for services and outcomes.

My introduction to eldercare came when I had two declining parents. I was working, raising kids, living across the country. I would fly in for emergencies, then spend hours on anxious standby waiting for the next crisis.

I came to understand that eldercare is a job for professionals. Few of us have acquired the extensive knowledge necessary to make decisions for a vulnerable parent or spouse. Family members are too emotionally invested and usually stretched too thin to resolve a loved one’s health challenges. Expert wisdom is needed from those who are educated and trained in supporting the needs of older people.

No one should try to keep an aging parent or ill spouse healthy and functioning without consulting a geriatric care specialists. It’s like caring for a child without guidance from a pediatrician.

“When a geriatric expert becomes the point person for all aspects of a loved one’s care, elders have a happy and joyful present, no matter their age.”

— Dr. Kathy N. Johnson, PhD, CMC

We work with people like you.

We are contacted by daughters, sons, spouses, relatives, friends, and others who find themselves in the role of caregiver. Our role is to guide older adults and their families through age-related challenges that cause emotional or psychological distress.

A safety net for elders without a partner.

There is much to consider when single and living alone. We specialize in preparing older adults on the medical, residential, financial and care realities of aging solo.

No need to leave home.

We meet with elders and their families wherever they call home, including assisted living facilities, hospitals, and rehab centers. In-person meetings provide a window into lifestyle, eating habits, medication compliance, mobility challenges, and more.

Getting started with a Care Manager.

Our first step is understanding your immediate concerns and the 4Ms (mobility, mind, medication, and what matters most to you). We then design a clear, actionable plan of care tailored to your individual needs and goals. This plan addresses quality of life issues, along with recommendations related to ongoing health conditions. We update the plan regularly to note changes and track results.

How professional eldercare makes life easier...

“The most important role of Care Managers is to act as a liaison between primary care providers and older adults. They sit in on medical visits, and are a second pair of eyes and ears.”

“I think the best thing about a Care Manager is having a point person for the patient and the family so now all parties have a face, a name, someone to call with questions or problems.”
